Sleep & CPAP Report — 2026-10-17
🚩 Flags
- 🟡 Flow-limitation metrics return to the window ceiling — Min_Above_020 10.1 min is the ABSOLUTE WINDOW MAX (rank 1/26 in high direction; z = +2.36 vs 30d; +83.2% vs 7d 5.5; +123.1% vs 30d 4.5). Edges past 10-10's prior high (9.8) by 0.3 min. Min_Above_010 26.3 min is rank 3/26 (z = +1.49; +36.4% vs 7d 19.3; +44.3% vs 30d 18.2), trailing only 10-10 (32.0) and 10-12 (26.7). FL_P95 0.13 is rank 3/26 in high direction (z = +1.44; +16.7% vs 7d 0.111; +28.3% vs 30d 0.099) — tied with 10-15 at the window's 2nd-highest reading. Min_Above_005 53.0 min is also elevated (+29.8% vs 7d 40.8; rank 2/26 after 10-10's 54.0). Yesterday's reversion (FL_P95 0.09, Min_Above_010 13.5, Min_Above_020 2.5) now reads as a one-night dip in an otherwise durably elevated FL regime. The 7-night FL_P95 path (10-11 → 10-17): 0.09, 0.14, 0.08, 0.12, 0.13, 0.09, 0.13 — only 2 of 7 nights at or below 30d baseline. The 'episodic FL' story is being overtaken by a sustained plateau.
- 🟡 Hypopnea count H = 8 — rank 2/26 in high direction (+100% vs 7d avg 4.0; +112.4% vs 30d avg 3.77). Highest H reading in the recent window — exceeds the prior recent peak of H = 6 (seen on 10-11 and 10-14). Combined with OA = 9 (rank 6/26; +31.3% vs 7d), the night's event total is dominated by hypopneas and obstructives, not centrals. 7d trailing H mean after tonight is 4.00, up from the 30d baseline 3.57.
- 🟡 AHI 2.65 — rank 6/26 in high direction (z = +0.81; +39.7% vs 7d avg 1.90; +38.8% vs 30d avg 1.91). Second-highest AHI in the recent window after 10-14's 2.86. 4-night AHI path (10-14 → 10-17): 2.86 → 1.42 → 1.86 → 2.65 — the 10-14 breakout is no longer looking like a one-night event; two of the last four readings are above 2.5. RDI 2.96 is rank 4/26 high (+45.4% vs 7d). The 2-of-4 elevated-AHI nights pair with the Min_Above_020 ceiling to form a clear 'elevated respiratory event night' profile, driven primarily by obstructive events plus the hypopnea spike.
- 🟡 Interesting divergence: FL and AHI are UP, but Pressure_Avg 7.20 is DOWN (-5.0% vs 7d 7.58; rank 2/26 asc — 2nd-lowest Pressure_Avg in the window after 2026-10-15's 6.98). Pressure_90th 9.66 is also below 7d (-4.3%). Normally rising FL/AHI drives EPR / APAP pressure up to compensate — tonight the device sat lower across the night despite elevated flow-limitation and event counts. This is the opposite of the expected feedback and suggests either: (a) events were clustered in a period the algorithm didn't see as persistent enough to trigger ramp-up, (b) the FL events were short and recovered on their own, or (c) a positional / supine-dominant pattern the lower-pressure baseline didn't address. Worth watching — if this profile (high FL, low pressure) persists for another 2-3 nights, it may be worth discussing with the sleep provider whether to raise the pressure floor.
- 🟢 CA = 0 — welcome break in the 3-night CA ≥ 1 streak (10-14 CA 3, 10-15 CA 1, 10-16 CA 2). 7d CA mean of 0.86 is still elevated vs 30d 0.37, but tonight's zero reading pulls the trend back toward baseline. If 10-18 is also CA 0 the recent CA uptick will read as a 3-night cluster, not a durable regime change.
- 🟢 TIB 390.38 min (6h 30.4m) — rank 19/26 asc (above median); +7.9% vs 7d 361.6; +3.5% vs 30d 377.1. First ≥ 6h 30m night since 10-14 (367.8 min); breaks the string of 4 short nights (10-12 335.7, 10-13 349.2, 10-15 328.2, 10-16 347.7). Sleep onset 22:56:16 CDT — the EARLIEST bedtime in the recent window (prior 4 nights ranged 22:32–00:02, so tonight is in-pattern on the earlier side). The variable-bedtime narrative from recent reports eases: the 5-night onset range is now 22:32–00:02, with tonight at 22:56 right in the middle. Total sleep 378.84 min (6h 18.8m), +7.6% vs 7d 352.1 — above both trailing baselines.
- 🟢 Sleep architecture stays rich-stage-dominant despite the elevated FL — Deep% 15.89 (rank 10/26 high, z = +0.62; +10.8% vs 7d; +16.0% vs 30d) + REM% 27.15 (rank 5/26 high, z = +0.50; +2.8% vs 7d; +9.4% vs 30d) = 43.04%, well above the 30d Deep+REM baseline (~38.5%). Deep minutes 60.21 is the 3rd-highest Deep-min reading in the recent window (behind 10-16's 58.71 and 10-07's 69.77). REM minutes 102.86 is above the 30d baseline but well off 10-14's 121.93 and 10-16's 121.94 records. The notable finding: tonight's rich architecture coexists with the window's FL ceiling — the FL-vs-deep% correlation across 108 confident matched nights remains weakly negative (-0.158), so an FL-ceiling night with Deep% above 7d baseline is a reminder that the FL/architecture relationship is weak at the population level.
- 🟡 Awake 11.54 min — rank 7/26 high direction (z = +0.40; +21.1% vs 7d 9.5; +24.8% vs 30d 9.2); sleep efficiency 97.04% is rank 6/26 asc. Third elevated-awake night in 5 (10-13 17.06, 10-16 13.55, 10-17 11.54). Pattern (10-13 → 10-17): 17.06, 5.02, 1.51, 13.55, 11.54 — an alternating but slightly rising awake-time baseline. Unlike 10-16 (which had clean CPAP metrics), tonight the elevated awake-time pairs with elevated FL/AHI — a plausible mechanistic story is that the FL events contributed to brief arousals. Across the 108-night matched set the FL_P95-vs-awake correlation is still -0.099 (negligible) — the population-level link is weak, but night-by-night the pairing is suggestive here.

Caveats
- Min_Above_020 at window ceiling (10.1 min) is the headline flag. It is a sharp outlier (z = +2.36 vs 30d) but by only 0.3 min over the prior ceiling (10-10's 9.8), so the practical significance is modest in absolute terms. The real story is the ensemble: three FL "minute-above-threshold" metrics in the top-3 of the window plus FL_P95 at rank 3/26. 4 of the last 7 nights now show FL_P95 ≥ 0.12 — the elevated-FL regime that tentatively formed 10-11..10-13 and seemed to break on 10-16 is now firmly re-established.
- Pressure-vs-FL divergence is unusual and worth monitoring. The APAP algorithm is designed to raise pressure when flow limitation persists. Tonight sitting at Pressure_Avg 7.20 (2nd-lowest in the window) while FL metrics hit a ceiling is counter-pattern. Could be a sampling artifact (FL events clustered in a time the algorithm didn't raise), or it could point to a sub-threshold event pattern that doesn't register on the device but shows up in the FL waveform. If the next 2-3 nights repeat the profile (low pressure + ceiling FL), this is worth bringing up with the ResMed clinician.
- H = 8 is the window max for hypopnea count. Combined with OA = 9, total event dominance is clearly obstructive (not central): 17 OA+H events, 0 CA. This is 2.3x the window's typical OA+H load (~7-8 combined). The RDI 2.96 reflects the respiratory event burden tonight exceeded most recent nights.
- CA zero tonight breaks the 3-night CA uptick. 7d CA mean remains 0.86 (elevated vs 30d 0.37), but tonight's zero reading is encouraging; the CA cluster (10-14, 10-15, 10-16) now looks like a transient response to the travel-return pattern, not a durable regime shift. Watch 10-18 for confirmation.
- Earlier bedtime (22:56) + longer TIB (6h 30m) is a positive schedule normalization. After 4 of the last 5 nights trending late/short, tonight returns to a more conventional pattern. 24-hour-equivalent sleep is approx 6h 18m from the main night only (no afternoon nap this time, per the single-session CPAP record).
- The rich sleep architecture (Deep 15.89%, REM 27.15%) persists despite the elevated respiratory and FL event load. This is a reminder that the CPAP → sleep-stage relationship at the night-by-night level is dominated by other factors (circadian, prior-night debt, environment) more than by the respiratory event count. Tonight is a case study: a 'messier' CPAP night still produced a clean sleep-stage profile.
- Matched-night confident count is now 108 (vs 107 after the 10-16 report). Correlations are stable at this sample size; the slight re-balancing of the Leak_90 median split (now 62 low / 46 high vs 46 / 61 last report — the 2.40 median flipped tonight's night into the "low" bucket) is a cosmetic artifact of the median-split method, not a signal.
- All flagged metrics remain within clinically safe ranges (AHI < 5, RDI < 5, no sustained desaturations, pressure well under 15 cmH2O max). Tonight is 'elevated flow limitation + bump in obstructive-event load, offset by rich sleep architecture and a return to normal bedtime.' The three-day arc (10-15 → 10-16 → 10-17) now reads: 10-15 short/variable, 10-16 recovery/REM-rebound, 10-17 longer night but elevated FL/AHI. The FL plateau is the item most worth watching if it persists.
